I want your petís experience here to be pleasant and as enjoyable as possible. It is important to understand that some pets respond to grooming differently, even with the best efforts. I will make every effort to make it a positive experience.

Sometimes pre-existing conditions arise unforeseen to me, such as skin allergies, shampoo allergies, moles, clipper sensitivity, matting and tangles, fleas, fear, behavioral issues, bone or joint sensitivity, heart condition, seizures, etc. It is helpful that you, as the pet owner, inform me of anything that you are aware of prior to grooming.

Matted coats and excessive undercoats cause a variety of health problems for your pet. Matted fur does not allow for air circulation to the skin, causing hot spots, bacterial and fungal infections. Fleas, ticks and other parasites may be lurking in the coat causing further skin infections. Matted fur also pulls and binds, causing pain to your pet when they move or lay on their mats. The skin underneath can be raw and inflamed.

Severely matted coats will require a short clip. I will be happy to show you how to care for your petís coat for the future, however, I cannot brush out a coat that is already severely matted due to the stress and the pain it will undoubtedly cause your pet. I may be able to remove excessive undercoat within reason and tolerance of your pet.

There is a chance that your petís skin will become more irritated during the stripping process. He/She may be nicked or cut because I have to work so closely to the skin to remove the coat. I am very careful, but the possibility

Of injury is there, and you need to be aware of this and give me permission to proceed.

I hereby release A DOGGIE IN THE WINDOW GROOMING STUDIO from any responsibility of the above mentioned process and any and all medical problems that may be uncovered and/or may occur during the stripping process.

Should my pet need veterinarian care during or after the process, I agree to pay any and all veterinarian fees for my petís care.

The undersigned pet owner or representative agrees to hold harmless A Doggie in the Window Grooming Studio and its owner, from any damage, loss or claims arising from any pre-existing condition and/or unrelated conditions of the undersignedís pet, either know or unknown to A Doggie in the Window Grooming Studio, without limitations.

In the event of an emergency, I authorize this establishment to obtain necessary treatment for my pet and agree to pay all such costs.

I have read the above policies and have discussed any concerns or questions with Cheri Rehr, owner, A Doggie in the Window Grooming Studio.

Client Grooming Agreement

Current Vaccinations/Veterinarian Information: By signing this contract, owners verify their pets are current on Rabies, Distemper and Parvo-Virus. Proof of Vaccination or current Titer testing shall be provided to 4 Paws Sake Pet Grooming upon request as well as current Veterinarian information. Special circumstances will be taken into consideration.

Aggressive or Dangerous Pets: Owners MUST inform 4 Paws Sake Pet Grooming if your pet(s) bite(s), has bitten, or is aggressive to people, other pets or specific grooming procedures. Muzzles may be used if necessary. Muzzling will not harm your pet, and protects both the pet and the groomer. 4 Paws Sake Pet Grooming reserves the right to refuse/stop services for such pet(s) at any time before or during the grooming process, and charge an Aggressive Dog Fee in addition to the regular grooming charge.

Health or Medical Problems & Senior Pets: Grooming procedures can sometimes be stressful, especially for a senior pet or pet with health problems, and can expose hidden medical problems or aggravate a current one during or after the groom. Because senior pets and pets with health problems have a greater chance of injury, these pets will be groomed for cleanliness and comfort, in styles that will not add to their stress. In the best interest of your pet this contract/agreement will give 4 Paws Sake Pet Grooming permission to obtain immediate Veterinary treatment for your pet should it be deemed necessary by 4 Paws Sake Pet Grooming. We will do our best to contact you first, then take your pet to your authorized Veterinarian or to the nearest Veterinarian that is available. It is agreed that all expenses for Veterinary care will be covered by the pet's owner upon signing this contract/agreement.

Mat Removal: Pets with matted coats need extra attention during their grooming session. Mats left in a pets coat only grow tighter, and can strangle the pet's skin, or eventually tear it open. 4 Paws Sake Pet Grooming does not wish to cause serious or undue stress to your pet, and will not continually de-mat your pet for you. Mats can be very difficult to remove, and may require the pet to be shaved. When necessary, removing a heavily matted coat includes risks of nicks, cuts or abrasions due to warts, moles or skin folds trapped in the mats. Heavy matting can also trap moisture and urine near the pet's skin allowing mold, fungus or bacteria to grow, causing skin irritations that existed prior to the grooming process. Torn skin from mats can also harbor maggots. After effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions and failure of the hair to re-grow. Shaved pets are also prone to sunburn and should either have sunscreen applied daily or should be kept out of the sun until the hair grows sufficiently to protect the skin. In some cases pets may also exhibit brief behavioral changes. Prevention is the best defense against matting by scheduling regular grooming appointments. There is an extra charge for dematting.

Accidents: There is always the possibility an accident could occur. Grooming equipment is sharp, even though we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In most cases this can happen when a pet is wiggling or moving around. If you arrive to pick up your pet and it is still being groomed, please do NOT talk to it or allow it to see you. Sit quietly, or step outside for a few moments. Every effort will be made to insure your pet is groomed as safely as possible, but an excited pet can be dangerous to continue to work on. If you insist on talking to your pet, or the groomer, we reserve the right to end the grooming session, even if the groom is not completed, and the full grooming price will be charged. Parasites: If you suspect your pet has fleas or ticks, prompt and thorough action on YOUR part is needed. Flea infestations can lead to tapeworm and other health problems. If fleas or ticks are found during the grooming process, your pet will be treated with a natural product to kill the parasites and will be charged for. Please note Ė 4 Paws Sake Pet Grooming will not use pesticide dips or sprays on your pet. Ticks found will be removed for an additional charge. If ticks are found, we strongly suggest you have your pet tested for Lyme Disease. Please note that parasites are a health hazard to your pet as well as to humans.

Hold Harmless Agreement: By signing this contract your (or your Agent) agree to hold 4 Paws Sake Pet Grooming, it's owners, operators, employees, officers and directors harmless from any damage, loss, or claim arising from any condition of the undersigned pet, either known or unknown to 4 Paws Sake Pet Grooming. It is also further understood and agreed the terms of this agreement can change at any time, without notice, and will overwrite any and all prior signed contracts or releases. It is further understood this clause applies to any and all pets groomed.

Late Pick-Ups: We do not have boarding facilities. If your dog is not picked up by closing for the day, and additional $50.00 kennel charge may also apply.

No-shows & Cancellations: No shows and last minute cancellations are subject to a $20.00 FEE PER PET which will be added to your next ticket. Pre-Payment for same will be required before another appointment is booked. We understand there may be emergency situations and will work with you, but not on a continued basis. Please be respectful of our time as we are a by-appointment business, and another client could have taken your appointment if we had known. Note: Clients 15 minutes late risk losing their appointment without notice.

Grooming Sedation Release Form

Please read the following carefully and sign. Your pet is scheduled for grooming to be done under sedation. Like you, my greatest concern is the well being of your pet. There are inherent risks any time anesthetics and sedatives are administered. The utmost care will be taken to try and avoid any complications but all complications cannot be foreseen. Animals that require frequent sedation for procedures (such as grooming, bathing, etc.) may be required to have periodic blood screening to assure kidney and liver health. I understand the risks of anesthesia and/or sedation and will not hold 4 Paws Sake Pet Grooming responsible for unforeseen complications.

MATTED PET RELEASE FORM


The need to remove matted hair is a necessity. When a coat becomes tangled beyond the ability to brush or comb through it, the most humane method is to shave off the matted hair. Shaving may seem severe, but it is the fastest and least stressful way to groom a matted pet.

Matted coats can cause the following problems:

*Skin irritation caused by dirt, dander, and parasites trapped under matted hair.

*Sores and hair loss: As mats get tighter, they eventually pull out of the skin, causing bald spots and sore skin. In severe cases, maggots can infest skin sores.

It is not uncommon to find preexisting conditions such as hot spots, sores, and other skin issues once the matted coat is removed. There is also a chance that your pet's skin may become irritated from being clipped so close. Ear bruising due to excessive head shaking after removal of mats can occur. Because the procedure takes extreme care and extra time, there will be additional charges depending on the severity of matting. A ‚Äúmat/ strip‚ÄĚ fee will be applied in addition to the regular groom cost ranging from five dollars to triple the regular groom price. All pets will be given an oatmeal bath and remoisturizing treatment to soothe skin.

I HAVE READ AND UNDERSTAND THE ABOVE. I hearby release Madra Pet Styling and staff from responsibility for the above mentioned shaving process and any problems that may be uncovered or occur during said procedure. I agree to pay any and all charges for said procedure. Should my pet require veterinary care during or after the procedure, I authorize Madra Pet Styling to provide necessary treatment for my pet and agree to pay any and all veterinary fees.

Roamin' Baths has informed me that there is always the slight risk of minor nicks when clipping off a heavily matted coat. This is due to unseen warts, moles and skin that can be pulled up into the mats. Please keep in mind this a rare occurrence and every reasonable, professional effort will be made to avoid this. Roamin' Baths has also explained that heavy matting can trap moisture near the skin which can promote fungal or bacterial growth resulting in a pre-existing skin irritation. Skin irritation can also result from self-inflicted damage by the pet (excessive licking or scratching) after the pet is shaved. Some pet's can also experience brief behavioral changes.

As the owner I am responsible for the condition of my pet's coat as presented to Roamin' Baths and agree to release and hold harmless Roamin' Baths in the event of aforementioned risks.

The skin under a severely tangled or matted coat can become/will be very irritated and sensitive. Under the matting your animal's skin is usually red, swollen, cracked, and dangerous to groom. There are higher chances of nicking or cutting the skin during dematting and/or clipping.

In the event that your pet is nicked or cut, your signature below authorizes Professional Pet Grooming to obtain veterinary care at your expense. Professional Pet Grooming will not be held responsible for a grooming injury to pets with matted coats. This also includes any after-grooming effects of matt clipping procedures, which could include, but are not limited to the following: itchiness, skin redness or self inflicted irritations/abrasions from excessive external rubbing.

I understand it is my legal responsibility to disclose, to the best of my knowledge if my pet has bitten, or attempted to bite, anyone in the past . I also understand that I, as the owner, will take full responsibility for all medical costs, as well as recovery costs, including physical therapy and lost wages should my pet ever injure anyone while on the premises.
